Stage one is raising an alert; anybody who has a duty of care to the individual has the responsibility to raise an alert. The person who has a duty of care should act to protect the adult at risk and deal with their immediate needs. They should also report it to their line manager and consider reporting to the police if it may be a crime. They also need to report this as there needs to evidence to support the individual if their needs to be an investigation further on. In Andrew’s case the doctors at A&E should raise an alert as he has attended A&E numerous times and therefore he is at risk. They should make sure they have all the documents to show when he has attended A&E, as they will need to show the social workers involved. Social workers will then take any immediate management action to identify and address the risk, they will then decide if a referral is needed. The safeguarding adults lead and the social worker has responsibility and will do an initial risk needs assessment. Andrew is a low risk case, as he is in no immediate danger, so they can refer him over to the in hours social worker.
