1. Elaborate on the CHN roles and functions in different community health settings.
Today, I was assigned to nurse Tonya. Things went a little different today. We started off the day with a meeting to inform every about what was scheduled today. Carlyle just hired a new director for the nurses, however, not much has changed. So there were several meetings lined up for the day. Here at the first meeting they informed every one of the clients status who were recently hospitalized. In the meeting they also inform everyone where they will be for the day. He the whole staff and community of Carlyle are involved.
In one meeting Tonya had argue her position on why the client could not receive the life term policy that guarantees a set rate throughout any placement in the assisted living community. This patient cleared had some neurological issues, however, some management did not think so because the family said otherwise. She was a little upset that they did not trust her assessment. She explained that she even went through each part of the assessment twice before moving on to the next steps.

Carlyle is a very interdisciplinary community. At Carlyle they have the counselors, nurses, patient care tech, social workers, sitters and everyone else works together to take care of the clients. Everyone know every person that is there. Even the clients take care of each other. The family members of the clients are very much involved in the lives of their loved ones.
