I would start by looking through their individual service plan to find out what the medical condition was, so I would have a rough idea what barriers I may cross. If the resident has dementia they may have a loss of their short term memory, this would mean repeating what I say to them. They may also need me to use simple sentence to be able to properly understand what it is I am saying to them.
Depression would be a barrier to communication. I would look in the residents’ individual service plan to see if they have a history of depression, also I would look at their medication to see if they are on anything to help with depression. I would also have a chat with the nurse on duty and identify how long the resident has been depressed for. Depression can cause many different moods, so when communicating with the resident I would look at their body language and facial expression to see what mood they were in. This would indicate if they are willing to open up and have a chat. Using simple sentences, touch, smiling, would help to comfort the resident and let them know you are there for them.
A sensory impairment would be a barrier to communication. Depending on the impairment the resident has, you would have to communicate differently. For example: with someone that has hearing loss you may need to change the tone of your voice to a lower tone, also speak louder for them
