Abstract
During the past few years, the Evidence-Based Medicine represents an integration of the clinical view of the patients’ safety. Throughout my years of being in the medical field, my Medical Knowledge grows every day. Evidence-based medicine is not easy to explain compared to medical practice. Because it requires a bottom-up approach that integrates the best external evidence with individual clinical expertise and patients’ choice, it cannot result in slavish, cookbook approaches to individual patient care. That is why I choose the five evidence-based performance for a few different scenarios related to prescribing medications to the right patients for the correct diagnosis. First Home Health Agencies will be the start and demonstrate five right for patients with five conditions that fall under steps of performance measures. Management of oral medication is a challenge for the patients.
