The applicant is a 32 year old male with no children, never been married. He is currently engaged to his significant other, who is a social worker and they have been together for 6 years. The patient is seeking readmission to CSAC as he was discharged in January of 2015 as he AWOL from the clinic and please note, the patient was at a 2 mgs before he could have received his final dose of taper based on the last discharge summary. The applicant reports, he's been clean from December 2014-February 26, 2016 until the tragic loss of his beloved brother at the tender age of 29.
