The claimant was hired permanently on 3/20/16 after she was hired temporarily as a Secretary for the City since July 2004.
The claimant alleges having pre-existing medical conditions and stress as a result of going through a divorce, financial difficulties, bankruptcy, and the death of her oldest daughter who was killed in 1998 in an automobile accident. The claimant says, as a result, her heath deteriorated from 2007 through 2009, when she was diagnosed with high blood pressure, left foot bunions, Fibroids in her uterus which caused her lower back pain. She said she had been under her doctor's care for these symptoms by Doctor Joseph Demattia.
On 1/12/2016, the claimant alleges she was called into the office by her Supervisor, Mr. Trng Huyhn and Ms. Jennifer Quan, Permanent Tech III where she received a formal reprimand for excessive absenteeism, tardiness and inattention to duty. She stated that was when she mentioned about having a Hernia. The claimant said she “felt it was not important enough to report her Hernia as a workers’ comp claim after she was diagnosed with it on 9/21/2015 through her doctor.” The claimant alleges she is had now reported her Hernia four months later because she believes her Hernia was contributed from the heavy lifting and transporting rolled up blueprints that weigh up to 75 pounds each, which is part of her job
