Clients Diagnoses (Clarita):
Clarita’s diagnosis is Reactive Attachment Disorder 313.89 (F94.1) probable (American Psychiatric Association, 2013). This diagnosis describes a pattern of withdrawn behavior toward her parents (mostly her step-father, but also her mother), an ongoing social and emotional disturbance characterized by limited positive affect (that is, she is not enjoying things she had once enjoyed), episodes of unexplained irritability (that is, aggressiveness towards step-father and step brother), lack of focus, ongoing depression, and nightmares that resemble suicidal ideation. Clarita’s recent visit to her father in Argentina may have produced the spark that increased symptoms of attachment disorder to a place where she can no longer cope with her situation. The visit likely triggered feelings of separation anxiety, and the reminders of her parents’ divorce that may have fostered guilt, especially if the Clarita believes contributed to the marital breakdown. Again, if the assessment of Clarita uncovers abuse or trauma at the root of her parents’ divorce, then this situation may change her treatment to focus on interventions related to post-trauma stress disorder and/or child …show more content…
She indicated that she did not want either the ex-husband or her current husband knowing about Clarita’s treatment needs. She is also concerned about what others will think of Clarita’s behaviour, especially since Terceira is working at Clarita’s school. From this, one can infer that she is concerned about Clarita’s behaviour being a reflection on her as a mother and/or impacting her status as an instructor. She may also be concerned about her ex-husband’s opinion of Clarita and his judgement on way that Clarita has been raised when living with Terceira. Terceira would benefit from identifying and accessing a variety of supports that can mitigate the negatives of her divorce, remarriage with step-children, raising adolescents, and information on attachment and mental health in general. She must monitor her own health to ensure she does not succumb to depression and stress. Her personal health and wellness is key to the successful treatment of
