Introduction:
My clinical placement is at the Home Care Office of Erie St. Clair. My role is Care Coordinator/Nurse. I will be completing 1.5 units for Nursing 495.This organization is responsible for provision of care under the direction of the Minister of Health. Our roles as Care Coordinators, and the eligibility criteria provided services are determined by the management team. This team consists of Chief Executive Officer (CEO), Senior Directors, Directors and Patient Services Managers. The Minister of Health and his team determines what home care can provide, how services are delivered, and manages the contracts with the service providers. Provincially the Local Health Integrated Networks (LHIN) have created internal service providers to address Chronic Illness and prevention of emergency department visits and hospital admissions. It will also ensure that by attaining this goal, I have endeavoured to provide quality teaching to my patients and their families.
The resources that I will explore are online articles and information through the Canadian Diabetes Association, and Canadian Nurses Association. I have also completed a literature review of articles on Diabetic Management in the community. I will ensure that I have reviewed the CNA Code of Ethics for Registered Nurses.
