EHRS can help identify certain patient populations at risk for specific diseases, for example, diabetics or those at high risk for developing diabetes (iHT Institute for Health Technology Transformation, 2012). Major risks factors or risk calculations can also be identified and populated. Individualized health plans can be coordinated based on the needs of the patient and even a population. Those at high risk for developing diabetes can be taught adaption of a healthy lifestyle such as glucose monitoring, proper nutrition, and exercise to minimize, and eliminate, the risk of diabetes.
