The purpose of this paper is to explore the outcome and effectiveness of clients and or patients currently in treatment. This paper will further examine various areas of client symptoms, quality of life, social role function, and mental health to name a few. In addition, this instrument also compares any changes over time from the beginning of therapy to the end of therapy. The results of this outcome may be further utilized in other assessments for additional evaluation of extended treatment or additional counseling.
