Mr. Ezekiel Green is a 72 year old male from Beaufort, SC. Mr. Green has been diagnosed with Hypertension, Type 2 diabetes, Neuropathy, hearing loss and poor circulation of the lower extremities. Mr. Green has had total knee replacement on both his right and left knees without any complications. Mr. Green was alert and cognitive function is good. Eyes, mouth, nose and ears look good. Respiratory and cardiovascular systems appear to be good. Normal “lub-dub” sounds of the heart. No wheezing or crackling sounds in the lungs. The abdomen is soft, non-tender without distention, no visible scars or lesions and visible pulsation. He does complain about constipation. Mr. Greens’ urine is a dark yellow color. He has no problem urinating and no frequent trips to the bathroom. Skin appears to have moisture, not dry, good skin texture, no lesions or bruising. The patient has great flexibility and is able to walk with assistance. Patient does have swelling of the lower extremities at the time of the assessment. Mr. Green does not drink any alcoholic beverages and does not smoke or use any tobacco products. Mr. Green states he goes to church and is a Christian.
