Mr. Gary Politte was referred to Blaine Rehabilitation Management, Inc. by Union Pacific Railroad for the purpose of providing vocational rehabilitation services. As part of my assessment process, I reviewed medical records, which included the following: Personal injury report from UPRR, Mercy Urgent Care; The Orthopedic Center of St. Louis; Premier Care; Neurosurgery and Neurology, LLC; Dr. Andrew S. Youkilis.

I met Mr. Politte at the DeSoto Public Library in DeSoto, Missouri, on April 4, 2016, at which time I completed an initial interview and conducted vocational testing. I did provide him with the Career Assessment Inventory (CAI) per our first meeting and directed his completion and submission for computer scoring. These testing results were received on April 19, 2016. As I had difficulty accessing the Internet (via a Mobile Hotspot) to complete all of the vocational testing per the first meeting, a second meeting with scheduled. Therefore, I met Mr. Politte again on April 29, 2016, at the library in High Ridge, Missouri.

Politte is interested in trying to return to Union Pacific and has been exploring options and even applied for current openings. He has even expressed a willing to relocate if this would provide a way for him to return to work. To date, he has been made aware of current security openings at Union Pacific and expressed an interested in learning more about them. Thus, to address his questions and interests, I notified Union Pacific and Mr. Polittle is scheduled for a teleconference with the Director of Disability Management to discuss current security openings and determine if this would be a realistic option for both Mr. Politte and Union Pacific. This conference call is scheduled for May 10, 2016 at 1:00 p.m. I will also plan to be on the teleconference
