Case Study (Extra Credit)

Part 1 Questions:

Are any of the lab values in Table 1 out of normal range? Do you see some that are too high or too low?
Many of the lab values in Table 1 are out of normal range:
The Serum Creatinine levels are too high (8.3 mg/dL)
The BUN levels are too high (55 mg/dL)
The Serum Calcium levels are too high (15.76 mg/dL)
The Serum Sodium levels are too low (128 mEq/L)
The Serum pH levels are too high (7.67)
The Urinary Potassium levels are too high (45 mEq/L)

Which of the lab values gives you information about how Mrs. Burroughs’ kidneys are functioning?
The lab values that give you information about how Mrs. Burroughs’ kidneys are functioning are the BUN and Serum

The family and I should be concerned about anything that Mrs. Burroughs takes that is not a prescription medicine because any medication, prescribed or over-the-counter, will alter the chemistry of the body. In addition, medication that is not prescribed can interact with other medicines, thereby hindering the functions of other medicines or by countering their functions.

Could any of Mrs. Burroughs’ current problems be related to the drugs (over-the-counter or prescription) she has been taking? Describe why you think there is a relationship.
Yes. The over-the-counter drugs could be related to the current problems Mrs. Burroughs is facing. I think there is a relationship between her problems and the over-the-counter drugs because she seems to be taking more than encouraged. In addition, these drugs can influence the function of her prescribed drugs. I do not think that her prescription medication alone has anything to do with her current condition because it was given to her by a doctor, and doctors are aware of implications drugs can have and their interactions with other drugs.

Burroughs’ breathing? What kind of change (if any) do you expect to see in the respirations of a person with metabolic alkalosis?
There is not a problem with Mrs. Burroughs’ breathing, but it could become a problem in the future. Changes that you would expect to see in the respiration of a person with metabolic alkalosis are hypoventilation or possibly apnea.

Can you draw a diagram that shows how the respiratory system, under the control of the central nervous system, responds to alkalosis?

Why do you think the physician suspected PTH levels that are too low?
I think the physician suspected PTH levels that are too low because Mrs. Burroughs’ serum calcium levels were very high. This is due to the fact that serum calcium levels and PTH levels are indirectly related. For example, if Mrs. Burroughs’ serum calcium levels were low, the parathyroid glands would secrete more PTH in order to raise blood calcium
