The health promotion model is relevant in nursing practice today because of it covers a life span and it is useful in a variety of settings (Alligood, 2014).The HPM the goal is wellness and the progression there of. When taking care of patients the HPM is relevant in all aspects of the patient overall care. Without realizing it, I have integrated the HPM in my daily practice and patient teaching. When providing discharge teaching the goal is to improve patient outcome and reduce readmissions. Thus improving patient health and disease prevention.
Nurses, should focus on health promotion and disease prevention with daily patient care. Patients are now sicker than ever and therefore Health promotion is critical for improving the life and well-being
