Patient X is currently in hospital due to having a fall at home and breaking his right femur, he stays at home with his mum, dad and baby brother. His mum and dad do not work so they are there all day with him, he also goes to nursery so has contact with other children. The objectives that help me to achieve this is to take his observation in the same time span each time so I will do this by taking every 2 hours, I have discussed this with the nurse. My goal for the activity is that his observation is checked to make sure that there are no changes in his charts.
