Health care reform is quickly changing the health care outlook. There are several ways to define hospital readmission, multiple stays within a specific time period, although the concern now is the cause of 30-day readmission. The common questions doctors and nurses get from patients “When am I going home?’. Getting discharged after a long hospital stay can be a celebration of patients and families being able to return to their own comfort. Hospital readmissions not only have the potential for negative physical, emotional, and psychological impacts on individuals in home health, and skilled nursing care, but also costing the Medicare program billions of dollars.
