Focus: Isaiah’s family will be able to teach and utilize self-care strategies to increase Isaiah’s independence. Ms. Smalls (MHP), Mr. and Mrs. Blakeney (MHS) and Isaiah discuss increasing Isaiah’s independence.
Intervention: MHP and MHS discuss Isaiah’s recent behaviors in the home and community. MHP encourage MHS to challenge Isaiah to become independent and not wait for others to get thing done. MHP question Isaiah about daily activities, plans for school and learning to care for himself. MHP, MHS and Isaiah review ways to improve self-care daily. MHP provide samples of changing small things around the home. MHP question Isaiah of resources given in the past weeks and the follow-up plan.
