1. All intakes for the HSI Safe Haven program come from the MHAMC-HOMI (Homeless Outreach for the Mentally Ill) group. The HOMI group/team manage the waiting list and only they have access to the waiting list. Referrals for homeless individuals with severe and persistent mental illnesses who are homeless or at risk of becoming homeless are directed to HOMI. Calls mad to the shelter are also redirected using a unique method.
2. When a call is made to the shelter, a phone screen is completed by a HSI staff who will then determine the program that the individual qualifies for. At this point all individuals with a mental health diagnosis such as bipolar, depression, PTSD and so forth will be forwarded to the Safe Haven program. Candidates may or may not be on medication to be forwarded to Safe Haven.
3. HSI Safe Haven case management will follow-up by calling the number indicated on the phone screen. Case management will ask candidates to visit the Morristown soup kitchen and Edna’s Haven and speak to a HOMI case manager. Supervisor will then after notify HOMI (Lisa, Chiffon and Ana) of referrals.

All contacts will be recorded on the HSI share drive with details on what was discussed. Feedbacks from MHA and outcomes (indications of being eligible or not for the Safe Haven program) will also be indicated. All outcomes will require verification. In the case that the individual is deemed ineligible for the Safe Haven program, he/she will have to give consent to the MHA case manager conducting the assessment to contact HSI. As soon as the consent gets signed, MHA will then disclose their assessment to the appropriate HSI shelter program. This allows the individual in need to proceed with intake at the other shelter programs. At this point the appropriate HSI case manager will follow up with a call to the individual with further
