As part of the team, my objective aligns with the organization’s goal of delivering superior care to our high-risk patients. The use of HCC recapture is crucial in this endeavor as it aids in identifying the needs of these patients. There was a specific instance that I noticed during a routine review of the CDAR list. I observed that a majority of those with high hospital readmissions were patients suffering from chronic conditions like uncontrolled DM2, CHF, HTN, COPD, ect. Some possible incentives that could promote work satisfaction are burnout prevention. Burnout prevention- In order to prevent APP burnout, there should be scheduled workshops in place for APP that teach them how to promote work-life balance. The workshop will emphasize on the APC Role and expectations, which will also include the roles and expectations of other team members with effective delegation techniques, avoiding a work environment that could potentially become toxic, stress management, daily exercises, and sleep schedule in addition to providing professional support for those APCs that struggle to attain the work life balance.
