Two weeks ago when I was in Intensive Care Unity (ICU) for my clinical rotation a patient admitted to the unit from emergency department. Wife due to weakness and fatigue brought this patient to the hospital. The patient had impaired cognition. According to his wife report generalized, the patient experienced some symptoms of fatigue and weakness for post a couple of days, but patient denied any pain, shortness of breath, and nausea but wife reported patient have some alerted level of consciousness prior to admission at a time. A patient had a history of clotting disorder and coronary artery disease. The diagnostic test had been done including CT scan of the head, and blood work since the patient had a history of blood clotting disorder. The CT scan of the head indicated acute encephalopathy suspect due to stroke and patient was diagnosed with stroke and admitted to ICU. I asked myself if this patient would not able to cough and not able to speak clearly. How are we going to perform a cough assist? I was able to recognize the problems, but I didn’t raise a question because I didn’t gather my evidence to support my solution to the problem also due to the level of my education I was shy to speak up with attending doctor at a time. What I was thought it would be a better alternative solution for this patient was to intubate the patient then it would much easier to clear the secretions by suctioning. However, even though I didn’t speak with a doctor to recommend this solution, but I communicated effectively with my clinical instructor to implement the solution for the better possible outcome for this patient. But as a critical thinker, we didn’t jump to conclusion. In medicine, we are supposed to gather evidence and have a collective data in order to support our
