IPE & VNA Summary:
Counseling activity: Counselor met with Jacob at STEP-South on 10/17/17, to have an initial meeting, to review the report from his high school, school to work classes, to discuss the results and recommendations from his summer work experience, to talk about his goal for employment, and to update his plan. To start this counselor introduced herself to Jacob and provided him with all her contact information. Jacob introduced himself and then the meeting proceeded.
To start, this counselor went over the last report received from the school to work classes that he participated in while attending high school. Jacob stated, “I liked working with Ms. Robin a lot and I feel like she taught me a lot of things.” Jacob proceeded to say that they school taught him a lot of skills and he feels like he is ready to work.
This counselor then went over the results and recommendations from the summer work experience, he participated in at Warren Woods Tower. Jacob completed a 3-week experience where he attended 5 days a…show more content… Jacob explained to this counselor that his mom was recently diagnosed with cancer and is pretty much confined to her bed. He stated that currently he stays with his aunt who is helping to take care of him. Jacob stated he would like this counselor to call and explain the MRS process as well as MCTI to his aunt, although he is his own guardian. He also stated, he wants to spend some time with his mom before he leaves as “the doctors aren’t sure how long she will live.” This counselor informed him that she understood his decision. She explained that she would call his aunt and continue to follow-up with him. After the holidays, she agreed to follow-back up with him and discuss MCTI. He thanked this counselor.
