Programs schedule employer evaluations at the end of a probationary period and then annually after that. It is helpful to ask the employer member to complete the form first, thereby allowing the ratings to show areas of agreement or discrepancy. New providers my be have a copy of the evaluation so that to know what the work expectations and can measure their own progress.
