LIVING ARRANGEMENTS
Jeffrey will be residing in a two family Individualized Residential Alternative (IRA) operated by Independent Living Association. The Individualized Residential Alternative (IRA) has a certified bed capacity of five Individuals. Staffing at the residence consist of 1 to 2 in the day and shared staff ratio in the night , a full time Residential Supervisor 20 hours a week, Assistant Residential Supervisor 40 hours a week, a Registered Nurse 5 hours weekly (on-call 24 hours), Residential Area Coordinator 40 hours a week, Behavior Intervention Specialist (Psychologist) 5 hours a week (on-call 24 hours), Maintenance staff 16 hours a week and Direct Care Staff 35 hours a week from 8am-4pm shared staff between both apartments

He likes his Day Habilitation Program because it offers him the opportunity to be more integrated into his community. While at program Jeffrey is working on the following valued outcomes: (1) To be productive and feel accomplished (2)To volunteer occasionally (3) socializing appropriately with others (4) occasional opportunities for community integration. Jeffrey enjoys assisting staff, setting the table for lunch, helping with the paperwork and at snack. He earns positive reinforcement when socializing and being respectful to his peers. Jeffrey has expressed an interest in volunteering at the Coney Island hospital. He continues to have a behavioral support plan in place which addresses maladaptive behaviors related to noncompliance and inappropriate behaviors. Jeffrey requires reminders to stay on task. He does not like to listen when spoken to. He also requires supervision due to his history of being inappropriate, such as using a high pitch tone of voice in the community. Jeffery has a history of fabricating stories, and being bossy. Due to inappropriate behaviors, he is not allowed to shop in several stores within the community. Jeffrey has a history of making unnecessary phone calls to certain Individuals at day program and other places. Jeffrey receives counseling with ILA Psychologist. Jeffrey continues to function at baseline. Jeffrey continues to become verbally aggressive and abusive when staff attempt to redirect him or intervene when he exhibits maladaptive behaviors. Jeffrey was evaluated to determine his status for sexual consent and it was determined that he cannot give sexual
