Jaylan's grandmother informed the QP, "Jaylan left with his cousin 2 days in a row and has not return home until the afternoon."

Jaylan needs assistance in improving his educational status, finding a job, and seeking medical and dental treatment before services terminate.
Jaylan denied having engagement issues; however, Jaylan is usually not available and leave home when visits are schedule in advance.

Jaylan established he is doing good with his emotional well-being, physical health, and living situation; however, he is doing fair with his relationship with family.
Jaylan does not listen to his mother, argues with her at times, come and go as he pleases, continues to smoke marijuana, hangs with the wrong crowd, hangs with gang members, and is not enrolled in school.
QP assessed Jaylan's symptoms have not decreased instead they have remain the same in except to his marijuana numbers fluctuating.
Jaylan's mother acknowledged, "Jaylan is not improving his life he is not doing anything during the time but hanging with his friends."
Jaylan's mother apologized to the QP for not returning her call because her phone service is not great; however, she agreed to meet next week to discuss Jaylan's options.
Jaylan's mother confirmed she would like for the IIH team to seek a 3rd authorization on his behalf.
