St. Charles Hospital in Bend, Oregon dates to 1918. The hospital was built by five Sisters of St. Joseph in hopes to care for everyone, no matter what (St. Charles Health System, n.d.). It is a nonprofit organization who has a close affiliation with the Catholic Church. The St. Charles health care system has over 220 medical providers.
Medication Error In December 2014, the St. Charles Hospital in Bend, Oregon had a hospital error. The hospital wrongfully administered the wrong medication to a patient in which ended to the patient’s death. This patient was Loretta Macpherson, a 65-year-old mother of two boys. Loretta Macpherson was originally obtaining services at the St. Charles Emergency Room after a pervious brain surgery at Swedish Medical Center in Seattle. Her physician at Swedish Medical Center prescribed Fosphenytoin, which is an anti-seizure medication. This medication must be given to the patient intravenously through a IV. The Journal of Patient Safety reported that preventable hospital deaths due to hospital errors array from 210,000-400,000 patients each year (CBS,2014). Inpatient hospital pharmacist find 10.5% of all medication orders received to be wrong (Tully & Buchan, 2009). Then of that approximately 11%, 7% of the error are potentially severe and may lead to death of the patient (Tully & Buchan, 2009). In most cases, pharmacist are the main personnel to locate and find medication errors.
