Maxcis Insurance Case

Ms. Castle of Maxcis Insurance Company referred this file for medical case management. Instructions were given to meet with Mr. Naranjo and assist with coordination of appropriate and related medical care to the right arm, and identify needs to facilitate recovery.

INTERVIEW SETTING

On 1/3/18 I met Mr. Naranjo at the office of Dr. Branch PMR. Mr. Naranjo is alert and oriented. He is willing to provide his medical history and the current status of the right arm. Mr. Naranjo is willing to work with a nurse case manager.

MEDICAL FACTORS AS RELATED TO INJURY

Mr. Naranjo said while he was at work he was rolling a truck forward onto a machine when he noticed a pop in the right arm. After a few days, he reported continued tenderness. He said he had a sharp pain whenever he performed a curling motion. Mr. Naranjo reported to his employer and was sent to RediCare. The Ultrasound report done by Sparrow hospital has also been included and was negative.

CURRENT STATUS

Mr. Naranjo reports he has pain and popping to the anterior portion of his right forearm. He reports that pain will radiate down to the wrist at times. He rated his pain at a 5. He is not taking any pain medications nor did he ask for any.

CURRENT TREATMENT PLAN

Dr. Branch reviewed prior medical dictation and also examined the right arm. Dr. Branch feels there is nothing surgical. He would like Mr. Naranjo to have an x-ray of the wrist and also to have an MRI to make sure nothing has been missed. In the meantime, Mr. Naranjo will also start occupational therapy. He was given several location options and was told to call and see where would fit in his work schedule.
