This was most often the case when the consumers did not know the answer. Most of the questions were general questions about the class, but I also added questions that were only applicable to each consumer and their situation. Therefore, if you read the notes they will appear the same, however if you read the entire note, you will find that not all the answers were identical. I explained to Mr. Rapp and also showed him how I set up my template and weekly progress notes to be recorded. I have been writing notes for seven years and have had weekly progress notes audits during at which time no superior expressed that I was "falsifying company documents by copying and pasting my weekly progress notes". Again, at no time did any auditor complain about how I was submitting my documentation. In addition, on February 22, 2016, staff were provided training on documentation from the COO Patrick Waters in which samples of current staff documentation were read aloud (including my own); and were provided feedback on what could make them better.
