Ultimately the person in charge and responsible for quality assurance falls to the director of nursing DON, and then as administrator I oversee it to make sure that every is been taking care on a timing manner.

Hard copy files on active patients are in a locked closed which is not open to the public and when the patient passes, we keep the record for those patients in another location.
14. What are the retention requirements for hospice records?
We keep them for at least thirteen months after the patient passes and after that for another three years.
15. Describe the role of HIM services in quality assurance activities.
Mainly a check and balance system. To make sure any issue is addressed we have a trigger tech system, so everyone who needs to know, have any update about a patient and we are in constant communication. Then again the check and balance the nurse go and visit and she makes sure that all documents are process correctly.
16. State Medicare HIM documentation requirements.
Medicare requires the legal packages, admission package, any time of visit must be documented in certain amount of time.
17. What statistics regarding hospice services are collected?
The HIS is statistics that we have to enter in on what is going on with our patients and we upload that and send it to CMS monthly and from there they compare it
