Statutory
Both- This provision is required by law. People receive personal care services free of charge (G.P). This includes people living in care homes and at home. Botha people with diabetes and hypertension attend to this type of care.

Both-this is the first time the patient goes to the setting do to the symptoms they are experience. A diabetic patient may be admitted for a short period of time to stabilise their condition with appropriate medication, followed by annual checks.
In a hospital there are varied facilities and because of that people have to be taken from one hospital to another for specialist facilities such as scans.
Home
Both- Most people who are ill prefer to be care for at their own home.
People with diabetes and hypertension are two of other disorders that will allow people to be acre for ate home until the later stage of the illness.
Social care setting
Both- when this both of this disorders (diabetes and hypertension) get the later stage of the illness a social care setting (such as residential care home) is necessary for the patient to go to, because their increasing disorder and so there is a need to keep them safe. However, there are some of them that can stay at home and still make it to the day-care setting several times a week for specific care (this is really good for the patient that have familiars that work during the day).

The treatment for diabetes role is to alleviate the symptoms (to maintain quality of life and slow the progression of specific and non-specific complications). The nurse’s role is to examine the person’s health and also to deliver care and its evaluation. This is also for hypertension.
Care managers
Diabetes- Care managers is a professional carer who carries out an initial assessment of care needs and then commissions a package of care to meet those needs.
The plan is to see what the individual is able to do independently and to prevent support where it is needed.
Social workers
Both- Social workers help people overcoming barriers and becoming full citizens with equal rights.
Pharmacists
Both- pharmacists help both people with diabetes and hypertension by dispensing medication. They are also allowed to give advice and some forms of treatment to the community.
For people with hypertension and diabetes pharmacists can be really helpful because many of them offer extra services such as blood pressure or blood glucose
