Outcomes-based practice focuses on improving well-being by understanding what matters to people and developing personal outcomes. It is essential that we gain a better understanding of the service users goals, values and beliefs in order to form a direction to the support we are providing. The aim of an outcomes approach is to make sure that people who receive care and support and their family or unpaid carers are supported to live the best lives possible, building on their own strengths and capabilities.
