Ellie is 10 years old Caucasian female. She resides with her mother, grandmother, great-grandmother, great-grandfather, and 3 brothers Travis, Zaiden, and Quentin. She does not take medication, has no medical condition, is aware of what drugs and alcohol and was able to distinguish the difference between the truth and a lie. She is seen at First Choice Pediatrics by Dr. Young.

She attends Howard Elementary and is in 6th grade. She likes to draw and listen to music videos. Ellie is outgoing and very friendly, and has lots of friends in school. She was observed to be smart, funny, and smiling at all times. She helps her mother with chores in the home, by doing the dishes and helping with the laundry.

Ellie is not diagnosed with any medical conditions or any mental health. Ellie was observed to be healthy and no concerns for physical discipline; no marks or bruises visible. Ellie seems to be happy and doing well in school. She does not show any emotional symptoms requiring intervention or lacks of behavioral out of control, and does not exhibit any self-destructive behavior that her mother is unable to manage. He is diagnosed with ADHD and takes Vyvanse (60mg) daily, and Guanidine .2mg daily for high blood pressure, and melatonin as needed. He has no other medical condition, no mental health, is aware of what drugs and alcohol and was able to distinguish the difference between the truth and a lie. He is seen at First Choice Pediatrics by Dr. Young every 3 months for his ADHD condition.
