INTRODUCTION
The purpose of this preliminary investigation report is to provide knowledge and better understanding of how an information system works in an organization such as Queensway Optometric Centre. With that purpose in mind we as a group interviewed a management level employee at Queensway to learn more about their current information system. To fully comprehend their system we did research on the organizations history, industry standards, competitions, as well as internal structure of the organization. Such as organization charts, reporting levels in the organization, level of decision making, etc. before doing the interview.
History of Queensway optometric Centre
Queensway Optometric Centre is a partnership, Dr. Patrick Tracy Built this Organization in 1988. A graduate of University of Waterloo School of Optometry, Dr. Tracey has practice in Mississauga since 1971, after 2 years in Peterborough, Ontario. He obtain his Therapeutic drug in 2001 at the University of Waterloo. Dr. Tracey has held several roles supporting Optometry in Ontario. An active member of the Ontario association of Optometrist, he has serve as a chairman of OHIP negotiating committee. He has also been a clinical associate at the UW School of Optometry. As a full-scope, primary care practitioner, he has special interest in sports vision and contact lenses. At present, he serves as one of the team Optometrist for the Toronto Argonauts of the Canadian football league and had previously spent time as part of the Vision care team for the San Jose Sharks of the national Hockey League and with the Oakland raiders of the National Football League. He then Started the Business with a colleague and Called it Queensway Optometric Centre at the Heart of Mississauga.

An organizational structure defines how activities such as task allocation, coordination and supervision are directed towards the achievement of organizational aims. It can also be considered as the viewing glass or perspective through which individuals see their organization and its environment. Organizations are a variant of clustered entities.
An organization can be structured in many different ways, depending on their objectives. The structure of an organization will determine the modes in which it operates and performs.
