Developing a Plan for Rehab Aftercare
A good rehab treatment program is one that follows up and through with rehab aftercare. The goals and requirements for this program must be individualized in the same way as the treatment program itself was. Your therapist must sit down and evaluate how you did while in treatment, as well as what your specific needs are.

However, this doesn't make them any less effective than when someone chooses to enter treatment by their own free will. When closely monitored drug rehab is always successful. This doesn't mean there isn't the opportunity to relapse. It simply means the chances of doing so are greatly decreased.
Some of the many different components involved in an aftercare plan include:
A prescription allowing you to consistently participate in a support or self-help group. Frequently known as a 12-step program, they include Alcoholics Anonymous and Narcotics Anonymous.
Regularly scheduled follow-up appointments for continued one-on-one therapy.
Required drug testing at times that you won't know about so the treatment facility can rest assured you're maintaining your sobriety.
Monitoring by scheduled appointments, phone, texting, video chat, or email so the facility can track your progress.
When necessary, this aftercare plan includes arrangements for you to live in a controlled living environment, halfway house, or sober living facility immediately after you're discharged from the inpatient treatment facility.
Your therapist should draw up your relapse prevention therapy so you can rehearse it together before you're allowed to leave rehab.
