This goal will be accomplished by completing the following objects establish programs to assist with obtaining and maintaining sobriety, casework services, and assisting the client with connecting with community resources. The outcome measures will be used to determine if this goal and objectives are being met. To complete the outcome measures the program director will collect baseline data during the intake process including the client’s basic information such as name, age, previous residence, relationship status, employment status and experience, and medical history. In addition, information regarding the client’s current needs will be gathered. This will include needs regarding public assistance such as food stamps and health insurance, clothing, and transportation to name just a few. Furthermore, specific information regarding the client’s substance use and abuse will be gathered. An already existing substance abuse inventory will be administered by program director during the intake process.
