Presenting Problem: Sabrina has 11 previous intake contacts for delinquent/criminal offenses. She is currently in custody of PWCDSS effective September 6, 2013. She was previously resided with her maternal grandmother due to severe substance buse issues and the criminal records of both biological parents. Grandmother reports Sabrina consistently disobeyed household rules. At the age of 10 Sabrina she began displaying concerning behaviors including running away from home, drug use and behavioral issues. It is reported tha Sabrina has been sexually active at the age of 11 and was diagnosed with Hepatitis C. In an attempt to correct behavior home based couseling services were put into place with Crossroads Couseling Center and her grandmother completed the Juvenile Justice Parenting Program, however behaviors continued to be an area of concern. Sabrina reports that she sometimes follows her delinquent peers in order to satisfy her addiction of Herion, Cocaine, Marijuana and alcohol. Sabrina reported beginning drug use at the age of 11 when she was in a relationship with a 20 year old man. Sabrina reported that she has not experienced any homicidal or suicidal ideations. She expressed that she has had suicidal thoughts and has attempted suicide. She reported that there is no Hx of physical abuse or indications of victimizations. Sabrina’s uncle reportedly sexually abused her at the age of 5 and again by another uncle when she was 11. Sabrina willingly takes responsibility for her actions and is fully aware of her actions. She can be verbally and physically aggressive in response to these acts of aggression. Academically her performance has been consistent maintaining a B+ average. Sabrina is in need of a secured and structured environment where she can obtain supportive services in order to be a functioning member of
