Bruises of a purple and bluish hue covered her arms but one was specifically bandaged with some type of device that appeared similar to a hand warmer. The patient was very frail and petite with a quite enlarged and swollen abdomen. Movement for the patient was very effortful and slow. After assisting the patient with movement to the commode it was clear to see that there was very poor skin integrity over her sacral area and she was at risk for pressure ulcers. Assessment of her lungs showed she had diminished expiration in her left lung. The patient was on oxygen 1 Liter per minute, and her nasogastric tube had suctioned out a thick very dark brownish green fluid filling about a fourth of the container. Pt had a 20g IV in her left forearm for Dextrose 5% in 0.45 normal saline. Other assessments were prevented because of personnel coming to collect blood work and complete a chest x
