Introduction:
In the past , the relation between the physician and the patient was like a father and " little" son , a lot of doctors were think that this “ paternalistic” .Also some doctors use another relation called " informed choice" and thought that the safety approach because the patient is solely responsible for the way of his/her treatment, but a new approach was invented to make the patient in center of the healthcare instead of the physician where patient has the right to know his/her problem and participate in the plan and management of his/her therapy, this approach known as Shared Decision Making.(SDM)
SDM is an approach where the healthcare provider along with the patient share the most suitable evidence for the patient when faced with task of making decisions which the patient is supported to participate in the options to achieve the best interest for the patient.(Légaré, F,2010)

The outlines of SDM is to give patient the right to share decision about his/her medical condition that the paternalistic model don’t give it to patient. Also it give support and recommendation to patient about best way of treatment that the Informed choice model don’t give it to patient. Models of clinical decision-making in the consultation can be represented as a spectrum from a paternalistic model at one end, to the informed choice model at the other end. In between these is the model of SDM . Author describes Paternalistic model : is when the physician is in the center of the decision making , where he give order for the patient to follow . the patient is subjected just to follow the order and act as a passive part in the decision making process
