The core measure chosen is the stroke core measure. This core measure set is made up of eight performance steps that must all be followed when handling stroke patients to ensure quality care. The organization strives to comply with the core measure set for stroke by following the eight steps. However, to date, the organization has only been able to follow five out of the eight steps. The five steps include VTE prophylaxis, patient to be discharge on antithrombotic therapy, given statin after discharge, give anticoagulants, and thrombolytic therapy. The guidelines will be provided to all clinicians at the hospital will the awareness will be provided in terms of education and training of physicians and clinicians. The education will be conducted by physicians and nurses who are aware of the core measures and understand the evidence linking the measures to improved patient outcome. The hospital’s management will also be affected by these changes as they will have the task of assessing how the changes are being implemented. Besides, the management will plan and finance the education and awareness about the core measures.
Plan for the changes
To implement the changes within the organization, the following plan will be followed:
Conduct workshops and training of nurses and physicians regarding the stroke core measures and the benefits of understanding and implementing them
Provide guidelines on how to implement the stroke core measures and steps
Initiate documentation and guidelines for stroke
