1. Describe the contexts of the systems that are related to your current place of employment (healthcare, education, community, political, social systems): I work for Summit Health and Rehab. Summit Health and Rehab has contracts with various skill nursing facilities (SNF), outpatient clinics, schools and hospitals in Louisiana, Texas, Mississippi, and Alabama. The facility I work in is Mary Anna Nursing Home. Mary Anna is a SNF that provides nursing care, occupational therapy, physical, and speech therapy to their residents. All therapist and therapy assistant must be licensed in the state of Louisiana, maintain continuing educational units, and meet service delivery requirements. Social: Mary Anna Nursing home and Summit Rehab does not allow any personal relationship with patients or co-workers. Summit prohibits employees posting anything on social media pertaining to the patients. There are no policies in place prohibiting being friends with patients or family members on any social media

Both female and males are residents. The age of the residents ranges from 46 to 103. Therapy is available for any resident in the nursing home that needs therapy. The common diagnosis that we treat is hip replacement, CVA, Dementia, and Parkinson disease.
3. Identify any trends in service delivery models –medical, educational, community, social – that may play a role in OT practice in your facility
Medical: Summit and Mary Anna keeps up with the changing policies of Medicare and Medicaid. The most recent change in Medicare was the change from ICD-9 to ICD-10. They both worked hard to help the change to be a smooth transition. We are also affected by Medicare cuts and Medicare Part B cap limits.
Educational: Summit and Mary Anna accepts physical, speech, and occupational therapy students. They also allow students to do observations hours at the nursing home.
Community: The most important goal for the majority of our patients is to be able to return home. We offer home evaluations for patients returning home. If they are planning to stay at Mary Anna, we gear our treatment for independence in the nursing home
