Microevolution is what has taken place in this situation as it was something that was only needed to be changed in a few people. As humans, we have the appendix which is sometimes mentioned to be something that we really don't need as an organ to help us function. They act as a safe house for good bacteria which can be used to effectively reboot the gut following a bout of dysentery or cholera. Basically it is something that we can live without as many people every year suffer from appendicitis, where the organ basically gets rid of itself and still live even though for a certain time death may seem near. Something I always thought to be weird is that if the appendix is so unnecessary, why are we still finding them in our bodies. I feel like one day this will stop developing in our bodies in the future because it is something that can honestly cause more harm to us than good. Something like this would be an example of macroevolution. Overall the behaviors or symptoms that a specific species may constantly participate in can cause the eventual evolution to either enhance or disable whatever may cause pain or success.
