Gianni Neil submitted a statement of incapacity/attending physician statement for Concurrent disability and leave dated 04/11/17. The claimant was reportedly unable to perform repetitive movements, heavy lifting (>10 lbs). Dr. Neil also stated that the Claimant has significant pain in the shoulder that has not completely resolved with conventional therapy because of continued use of joint. The form indicated that the claimant stated that the complaints since the new assignment of washing heavy pots and more repetitive movements. She started having shoulder pain on right side. The diagnoses were rotator cuff disorder and osteoarthritis. Dr. Neil also mentioned that the Claimant’s primary diagnosis is resulting in severe, deliberating pain and the expected time to rehabilitation this injury is 3 months. Care was initiated on 10/17/16 and first diagnosed with this condition. The diagnoses were disorder of synovium and tendon and osteoarthritis. All these injuries are resulted due to injury in July 2016, when she had a change in job to more and repetitive lifting. She was recommended for orthopedic surgeon visits and physical therapy 2-3 times per week.
