TOTAL HIP ARTHOPLASTY 1
Abstract
Total hip arthroplasty is the medical term for total hip replacement. A total hip arthroplasty is the surgical procedure of removing the damaged cartilage, acetabulum, and femur. Then replacing both bones with proper fitting prosthesis. This procedure can be needed for many reasons such as trauma, osteoarthritis, avascular necrosis, bone tumors, or osteomalacia. Lately his symptoms have been getting worst and his medication is not helping. Billy Bob have been experiencing severe pain in his hip. Joint stiffness when he’s getting out of bed, or walking after sitting for a long period of time. Swelling, and tenderness around his hip and groin area. Also sometimes he hears a “crunching” of bone rubbing together he says. When he went to an orthopedic surgeon a hip examination was done including x-rays, and a physical test. The examination results showed that his bones were rubbing together from lack of cartilage and that his joint were inflamed that’s why he heard the “crunching” sound. The surgical site will be prepped and draped with the aid of resident, attending, circulator nurse, and surgical technologist. The surgeon will come in after this step is done and gown and glove. A time out will be done to make sure they have the right patient, surgical site, known allergies, and plan of procedure. This is done to ensure the patient’s safety. Next, an incision will be made just behind the hip, along the otter buttock area about 4-6 inches long with a #10 blade. Self-retaining retractors such as Gelpi, or Weitlaner will be used. The surgeon may want the attending or resident to help retract and have them use hand held retractors such as rakes, or Richardsons. It really depends on the surgeon’s preference.
