Total Knee Replacement is an elective surgical intervention that effectively relieves pain and improves function in persons with symptomatic end-stage knee osteoarthritis (OA) and post-traumatic arthritis (PTA).The procedure was develop to relieve pain and improve function in the middle age and elderly population. With the demand for TKA increasing each year, a projection study that ranged from 2005 to 2030, identified that the rate of TKA performed will increase by 648% being that 450,000 procedures were performed in 2005 which is expected to increase to 3.48 million procedures in 2030.Kurtz

Before a TKR is performed, many factors are evaluated to determine if it the most appropriate intervention to reduce pain and improve function. A study

Fisher,Bourne A study identified an association between poor surgical outcomes with patients who were younger in age, female, higher BMI, had a history of depression and previous knee surgeries.Fisher A physical impairments that is often seen is arthrofibrosis, or stiffness, which results in limited knee ROM. Fitzsimmons A stiff knee is considered to be a lack of 15 degrees of knee extension or knee flexion less than 90 degrees Fitzsimmons In some cases, if the ROM limitation is significant enough, it can lead to a manipulation under anesthesia (MUA) procedure to regain motion. Knee flexion ROM of 83 to 106 degrees is considered to be functional for daily actives such as stairs and bending down to tie shoes, but anything less than 83 degrees could negatively affect a person’s quality of life. Laubenthal It is recommend and most beneficial that the MUA procedure take place within 3 months of the original surgery, although it has also been shown that there are still benefits to the procedure even if it is performed after the 3 month window. Fitzsimmons Patients at risk of developing stiffness resulting in a MUA are younger, likely to smoke, or have had a surgery on the same knee.
