Throughout the years health management has improved in many ways but it also has its issues. On the following paragraph I will summarize the issues according to the author of the article in Health Affairs. Then I will discuss my thoughts on any management or motivation theory I think could be applied to the issue. After I discuss these steps then I will talk about the courses I have taken that provide theories, practices, or application for this real world situation. The article titled The Importance Of Transitional Care In Achieving Health Reform by Mary D. Naylor and Linda H. Aiken, mentions interesting topics relating to transitional care model, Affordable Care Act, quality of care, caregivers, Medicare savings programs, organization of care, and others (Naylor, 2011).
Transitions, or “handoffs,” are vulnerable exchange points that contribute to unnecessarily high rates of health services use and health care spending (Naylor, 2011). And they expose chronically ill people to lapses in quality and safety. It is during these transitions that mistakes commonly

The Affordable Care Act also supports transitional care services through a number of other vehicles. The Center for Medicare and Medicaid Innovation (Section 3021) offers an unprecedented opportunity to identify, evaluate, and disseminate innovative care delivery and payment models, including transitional care, through a $10 billion allocation for the period 2011–19 (Naylor, 2011). Under the Medicare shared savings program (Section 3022); moreover, accountable care organizations will be required to submit performance data that may address care transitions across health care settings (Naylor, 2011).
