was referred for a neuropsycholgical evaluation by Daryl Lamb, MD. Susan reported that her daughter committed suicide (4/8/2014). In addition, Susan reported that she is unable to do anything for very long. She does not believe she can work in any setting. Susan stated that she could maybe work “two hours”, but she has physical problems as well. Susan believes that her doctor wanted her to get a neuropsychological evaluation to document her total functioning deficits that prevent her from obtaining substantial employment. Therefore, a primary purpose of the evaluation is to determine if neuropsychological factors may be contributing to Susan’s difficulties, and to provide appropriate diagnostic and treatment recommendations. Finally, Susan reported that this case is involved in litigation “for disability”. Her attorney is “Ober & Pecas”.

Her reason for this appointment was a follow-up visit. It was reported that Susan continued to take her Atorvastatin, but would like to stop taking it, due to her dietary changes (i.e., she cut out meat, dairy, processed food, and sugar). It was reported that her cholesterol was half of what it was previously. Susan had not done any physical activity, but stated her fibromyalgia improved since her last visit. Her assessments suggested issues in the following areas: hyperlipidemia (primary); hypertension; fibromyalgia; GERD; insomnia; and Vitamin D deficiency,
