Pa requires consistent cueing, coaxing, encouragement, support, supervision with daily tasks of bathing, dressing, grooming, mobility tasks, and meals. Pa receives medication management from sister Gilda along with ADL, IADL, mobility assist and supervision. Pa’s prefers sister to be PAS. Pa primary goal is to remains at home safely. Pa will receive the service plan in the following type, scope, amount, frequency and duration, Agency Model and Pa directed; 7 days per week x 3 hours 9:00 am – 12pm. PAS will provide supervision and hands on assistance with bathing, dressing, grooming, meal prep, light housekeeping and laundry. Pa has access to emergency medical responder via Medscope. Pa will also receive Service Coordination from PCA; Pa agreed to a contact plan of visits every 3 months and call during non-visit months. SC will complete an annual clinical reassessment. Information on self-directed services was provided to Pa, Pa prefers son to be aide. SC reviewed prohibition of restraints and restrictive interventions with Pa; he was informed that he should call his SC or the OLTL Helpline to report unauthorized use of restraints or seclusions. SC reviewed the transportation services that are available; Pa does not currently utilize any transportation
