The goal of case managers is to help those who need assistance manage their lives and to offer support in crisis (Woodside & McClam, 2014). Case management has three distinct phases; assessment, planning, and implementation. Each process serves as the foundation to helping clients effectively and efficiently. When approaching case management for hospice and palliative care patients, Ms. Vigo suggests that these, are the most critical pieces to the puzzle in her professional role.
Phase One of Case Management
The process begins when initial contact is made with the client. This is the onset of the helping relationship. The client is the primary focus must be involved in and all aspects of care. Engaging with the client to building a rapport,
