I have spoken with Mr. McDonald and the physical therapist Dave Thibdeau. Mr. McDonald was working and making progress in his work reconditioning program until mid April. He had an increase in low back pain. He required an extra Chiropractic adjustment. He then went on vacation for a week and then we had an appointment with Dr. Shah on 5/2/17. Mr. McDonald feels he is about 60% better. His core strength is better. He still takes Motrin only for pain. His wife reports he has a difficult time sleeping at times. He continues to use the TENS Unit, cold and heat. After examination Dr. Shah agreed to add Norco to take at bedtime. He also agreed to continue all of the current services. He will want Mr. McDonald to have a FCE prior to returning to work when the work reconditioning program has been completed. I have updated the physical therapist. McDonald is off work until 9/21/17. I have updated Cherry Grant, employer on 4/20/17, 4/25/17 and 5/2/17. There is no light duty work to return to. I have requested a job description which was provided to physical therapy.

ASSESSMENT

Mr. McDonald made good progress up until 2 weeks ago. He will remain in the work reconditioning program. He remains hopeful that he can return to work.

PLAN/RECOMMENDATIONS

1. Contact Mr. McDonald every 2 to 4 weeks to discuss locations for chiropractor, acupuncture, and the fitness trainer.
2. Attend the next appointment with Dr. Shah on 9/21/17. Determine if Mr. McDonald is ready to return to work.
3. Update employer on current work status after doctor
