Working in a low-income poverty striking community, where medicine is scares to those who could not afford it; my 15-30minutes acts of kindness, interactions, and feeling my patients pain goes along way. To me, this is empowering and innovating to the community to whom I serve, keeping in mind that he/she could be me. Nonetheless, there are many areas of healthcare that needs change. One that came to mind and is troublesome in healthcare today is the patient noncompliance to medical plan/regimen. Most patients hate the idea of taking more than one medications (polypharmacy) or even seeking medical. This to me is one of many areas identified as needing change. To help in this matter, my current employer has adopted the Patient Centered Medical Home (PCMH) as an innovative tool to help increase overall compliance to medical care. PCMH is a great innovating process that helps channel in medical care and communication to transform primary care services into patients’ own homes. This would lead to higher quality compliance rate, lower cost, and improve patients’ and provider care
