The family vision is for Melina to learn appropriate ways to address her mental/behavioral health needs so that she is able to build self-confidences and reframe from unsafe behaviors.

Family: Melina is a 16 year old Hispanic female who lives in Elizabeth with her mother Ms. Pierona Ventura. At this time Melina is doing well in the home since her discharge from the Trinitas Hospital PHP back on 3/13/18. Due to Mrs. Ventura mental health challenges at this time she is unable to care for youth. Mrs. Ventura has made temporary arrangements for Mrs. Amalia Caputi a family friend to care for Melina until she is mental stable and is able to care for youth again. Mrs. Ventura mention to CM about going to DR to clear her head but she could not give CM a time

She express to CM that she does not want to go to DR in the middle of her junior year. Youth has inconsistent contact with her biological father who lives in DR.

Action: CM will monitor youth progress while she is in Mrs. Caputi care.

Psych/Mental Health: Melina diagnosis are F39, Unspecified mood [affective] disorder; F40.10-Social phobia, unspecified; F43.20-Adjustment disorder, unspecified; F60.3-Borderline personality disorder and F90.0-Attention-deficit hyperactivity disorder, predominantly inattentive type. She is currently on Melina started her news medication Effexor XR 37.5 mg. Melina attended the Trinitas Child & Adolescent Partial Hospitalization Program back from 2/27/18-3/13/18.

Youth successfully completed the Trinitas PHP. It was recommended by Mrs. Coyle, Program SW that Melina returns back to school, continue to attend the YES Program at Thomas Jefferson Arts High School and attends the Trinitas the Trinitas afterschool program Monday through Friday. Mrs. Coyle also made a referral to the Trinitas Child & Adolescent Department DBT group for Melina. Melina was placed on the waiting list for the DBT group. The plan was for
